3 Tantangan Penanganan Diabetes di Indonesia, Termasuk Keterbatasan Akses Pengobatan
KOMPAS.com – Di Indonesia, jumlah penderita diabetes terus meningkat dari 10,7 juta pada tahun 2019 menjadi 19,5 juta pada tahun 2021.
Tahun ini, Indonesia menduduki peringkat kelima dengan jumlah penderita diabetes terbanyak di dunia, naik dari peringkat ketujuh tahun lalu. Para ahli menganggap bahwa peningkatan prevalensi diabetes ini sangatlah memprihatinkan.
Dari dua jenis diabetes yakni Diabetes Melitus tipe-1 (DMT1) dan Diabetes Melitus tipe-2 (DMT2), DMT2 menjadi yang paling banyak menyerang lebih dari 90 persen pasien di seluruh dunia. Diperkirakan terdapat 1 orang meninggal setiap 5 detik akibat diabetes ini.
Peningkatan jumlah penderita diabetes tipe-2 (DMT2) disebabkan oleh faktor sosio-ekonomi, demografi, lingkungan, dan genetik.
Beberapa pendorong utamanya adalah urbanisasi, populasi yang menua, berkurangnya aktivitas fisik, dan peningkatan jumlah masyarakat yang menderita obesitas atau kelebihan berat badan.
Jika tidak ada intervensi, angka kasus baru dan kematian akibat diabetes ini diproyeksikan akan terus meningkat.
Kematian akibat diabetes ini bisa terjadi karena adanya komplikasi pada pasien tersebut.
Komplikasi diabetes memang berkembang secara bertahap, tetapi ketika terlalu banyak gula menetap dalam aliran darah untuk waktu yang lama, maka hal itu dapat memengaruhi pembuluh darah, saraf, mata, ginjal dan sistem kardiovaskular.
Berdasarkan catatan Kementerian Kesehatan Indonesia, setelah 10-15 tahun dari waktu teridagnosis, prevalensi semua komplikasi diabetes meningkat tajam.
Berikut beberapa komplikasi diabetes yang bisa terjadi.
– Retinopati diabetik (gangguan mata atau penglihatan)
– Penyakit jantung dan pembuluh darah
– Serangan jantung
– Stroke
– Nefropati diabetik (gangguan ginjal)
– Neuropati diabetik (gangguan saraf yang menyebabkan luka dan amputasi pada kaki)
– Infeksi kaki yang berat
– Gagal ginjal stadium akhir
– Disfungsi seksual
Tantangan yang harus diatasi
Dalam upaya menurunkan prevalensi kasus baru dan kematian akibat diabetes melitus di Indonesia ini, ada tiga tantangan utama yang harus segera diprioritaskan untuk segera diatasi.
1. Keterbatasan akses pengobatan
Keterbatasan akses pengobatan diabetes pada akhirnya menghadirkan banyak tantangan lainnya dalam penanganan penyakit ini.
Berdasarkan data Center for Health Economics and Policy Studies (CHEPS) Fakultas Kesehatan Masyarakat Universitas Indonesia (FKM UI) dan Perkumpulan Endokrinologi Indonesia (PERKENI) pada tahun 2016, pemerintah menghabiskan 74 persen anggaran kesehatan untuk secara khusus mengobati komplikasi diabetes.
Padahal, komplikasi diabetes dapat dihindari dengan mengontrol tingkat gula darah.
Diperkirakan masih banyak orang dengan diabetes yang tidak terdiagnosis. Masih banyak juga orang dengan diabetes yang tidak berhasil mencapai target HbA1C yang sudah ditentukan atau tidak terkontrol.
Studi menunjukkan, lebih dari 70 persen orang dewasa dengan diabetes melitus tipe-2 di Indonesia tidak berhasil mencapai target HbA1c di bawah 7 persen.
Jumlah diabetisi dengan gula darah tak terkontrol terus meningkat dengan mengkhawatirkan.
Orang dengan DMT2 biasanya membutuhkan insulin ketika diet, olahraga, dan pengobatan antidiabetes oral masih tidak membantu mencapai gula darah yang ditargetkan.
Insulin adalah bagian dari manajemen diabetes dan bukanlah akhir dari hidup. Insulin semestinya akan membantu orang dengan diabetes untuk mengontrol gula darah, agar mengurangi risiko komplikasi.
Insulin juga merupakan obat diabetes pertama yang telah menyelamatkan banyak jiwa.
Meski demikian, banyak masyarakat Indonesia yang tidak menyadari ancaman DMT1 pada anak dan remaja yang dapat berdampak seumur hidup.
Hal ini terlihat dari tingginya tingkat underdiagnosis dan misdiagnosis DMT1 pada anak-anak, serta masih tingginya jumlah anak yang menderita ketoasidosis diabetik (DKA)
Anak-anak penderita DMT1 yang tidak memiliki akses ke layanan kesehatan akan sulit mendapatkan hidup sehat, mengembangkan potensi dan bakat, serta mewujudkan cita-cita mereka.
Duta Besar Denmark untuk Indonesia, H.E Lars Bo Larsen mengatakan, tahun ini peningkatan jumlah orang dengan diabetes yang pesat menjadi peringatan mengenai pentingnya kesehatan masyarakat, terutama pada masa pandemi Covid-19.
Larsen juga menambahkan, World Diabetes Day tahun ini dapat menjadi kesempatan untuk berbagi informasi mengenai pencegahan diabetes dan meminta tambahan sumber daya dan perhatian yang lebih memadai.
“Kami meningkatkan kolaborasi dengan pemerintah Indonesia dalam meningkatkan kesadaran diabetes dan menyediakan akses pengobatan bagi penderita di Indonesia,” kata Larsen
Akses pengobatan diabetes sangatlah penting, karena orang dengan diabetes memerlukan perawatan dan dukungan yang berkelanjutan untuk mengelola kondisi mereka dan menghindari komplikasi.
2. Edukasi mengenai diabetes
Tantangan untuk menurunkan prevalensi diabetes berikutnya adalah pemahaman masyarakat per individu mengenai pentingnya mencegah, mengatasi, dan mengobati segera penyakit diabetes.
Hal ini dikarenakan masih banyak sekali masyarakat yang ternyata enggan untuk memeriksakan diri, karena khawatir jika mereka benar-benar didiagnosis menderita diabetes.
Pengalaman ini, salah satunya disampaikan oleh Helma Rosa (40), warga Kota Palembang, Sumatera Selatan. Kepada Kompas.com, Kamis (2/12/2021) Ema menceritakan bahwa dirinya sudah sekitar 8 bulan belakang sering sekali merasakan keinginan buang air kecil yang tidak tertahankan.
“Ayuk (panggilan kakak perempuan dalam bahasa Palembang) tuh sering nian (sekali) kalau pengen kencing itu dak (tidak) bisa nahan. Jadi, kalau ado raso nak (ada rasa mau) kencing ya sudah harus kencinglah saat itu,” kata Ema sapaan akrab Helma Rosa ini.
Bahkan, lanjut Ema, keinginan buang air kecil yang tidak tertahankan itu dalam sehari mungkin ia bisa sampai 60 kali lebih.
Kalaupun sedang melakukan perjalanan dengan kendaraan roda empat, jika ada keinginan buang air kecil, mobil tersebut dipaksa harus berhenti entah itu saat sedang dalam tol sekalipun.
“Ayuk tuh dalam sehari mungkin ado 60 kali lebih nak pipis (buang air kecil). Memang sih ayuk banyak minum jugo (juga),” cerita Ema.
Ternyata dalam sehari, Ema bisa menghabiskan lebih dari 3 liter air minum dan kebanyakan adalah air es dingin baik itu air segar biasa maupun air dengan tambahan gula, seperti es teh manis.
Ema menyadari bahwa sebenarnya ada yang salah dengan kebiasaan keinginan buang air kecil berlebihan atau beser yang dialaminya itu. Tetapi, ia takut mengetahui dirinya kalau nanti sampai didiagnosis diabetes.
“Sebenarnya dak tahan beser cak ini (begini) terus. Tapi, ayuk tuh takut periksa kalau gek (nanti) hasilnya diabetes. Mending ayuk dak (tidak) tahu,” ujarnya.
Untuk diketahui, para ahli menyebutkan bahwa meningkatnya frekuensi buang air kecil memang bisa menjadi tanda awal dari penyakit diabetes.
Selain Ema, cerita perihal diabetes ini juga disampaikan oleh Ana, masyarakat di Musi Banyuasin, Provinsi Sumatera Selatan.
Berbeda dengan Ema, Ana dalam hal ini menceritakan kisah ayahnya yang sudah meninggal akibat diabetes.
Diceritakan Ana (35), Ayahnya (B) telah meninggal dunia 3 tahun lalu karena mengalami infeksi dalam waktu lama pada kakinya yang tertusuk paku, dan tidak bisa disembuhkan karena penyakit diabetes yang diderita.
Menurut Ana, almarhum ayahnya menderita diabetes karena kebiasaan mengonsumsi minuman teh cangkir kemasan berperasa setiap hari, di sela-sela kerjanya sebagai pengepul sampah bekas.
Dalam sehari bahkan almarhum ayah Ana bisa menghabiskan minuman cangkir kemasan itu sampai 4-5 buah. Meskipun sudah ditegur oleh anak-anaknya untuk mengubah kebiasaan itu, almarhum ayah Ana masih tidak peduli.
“Iya baba tuh kebiasaan minum teh gelas hampir tiap hari. Makanya ada diabetes. Waktu kakinya terpijak paku karatan, jadi infeksi dan susah sembuhnya, bertahun-tahun itu sudah (infeksi tersebut),” ujarnya kepada Kompas.com, Sabtu (4/12/2021).
Infeksi yang berat pada kaki dan gangguan saraf yang terjadi memang merupakan salah satu akibat dari komplikasi diabetes yang ada.
Seperti diketahui, kebiasaan mengonsumsi panganan yang tidak sehat dan mengandung gula, malas bergerak atau olahraga, juga riwayat genetik keluarga merupakan beberapa di antara faktor risiko penyakit diabetes.
Sayangnya, setelah mengetahui ayahnya memiliki riwayat diabetes, Ana dan saudaranya belum pernah melakukan deteksi dini diabetes.
Mereka beranggapan selagi tidak ada gejala, dan menghindari konsumsi minuman kemasan instan yang manis-manis, mereka bisa terhindar dari penyakit diabetes.
Dua cerita di atas mungkin hanya secuil dari cerita banyak orang terkait diabetes di Indonesia.
Oleh sebab itu, Vice President & General Manager Novo Nordisk Indonesia, Anand Setty mengatakan, edukasi tentang diabetes ini menjadi tantangan yang sangat penting.
“Kami ingin menjangkau sebanyak mungkin orang dengan diabetes di Indonesia dengan melakukan berbagai kampanye dan edukasi mengenai diabetes, bergabung dengan gerakan global Changing Diabetes in Children, menyediakan berbagai produk inovatif, dan mendukung pemerintah dalam berbagai pendekatan yang bersifat preventif,” kata Anand.
Selain itu, Anand juga mengatakan, Novo Nordisk akan melakukan kolaborasi dalam hal chatbot diabetes.
“Masyarakat bisa mendapatkan informasi mengenai diabetes melalui platform WhatsApp. Kami berharap chatbot ini dapat menjadi sumber informasi utama mengenai diabetes,” jelasnya.
3. Pentingnya akses obat insulin
Insulin adalah penemuan yang menyelamatkan hidup (life-saving discovery) bagi orang dengan diabetes.
Bagi sebagian orang dengan diabetes, terutama diabetes tipe 1, insulin membantu mereka untuk beraktivitas seperti biasa dan mendapatkan kehidupan yang berkualitas.
Insulin bukanlah last resort atau pilihan akhir, melainkan salah satu upaya yang paling efektif untuk mencegah terjadinya komplikasi.
Sehingga, tidak boleh menunggu ada komplikasi baru menggunakan insulin.
Masalahnya, akses pengobatan insulin masih menjadi polemik tersendiri bagi masyarakat.
Hal ini yang dialami oleh Oto (65), warga Kota Bekasi, Jawa Barat. Ia menceritakan bahwa dirinya memiliki beberapa gejala diabetes, tetapi hanya bisa mengonsumsi obat oral pengendali gula darah, bukan insulin.
Hal ini dikarenakan, selama ini dirinya cukup sulit untuk mengakses BPJS di Fasilitas Kesehatan Tingkat Pertama (FKTP) setempat.
Sebab, mereka tidak memiliki kartu BPJS, meskipun Oto dan keluarganya termasuk penerima bantuan jaminan kesehatan BPJS yang iurannya dibayar langsung oleh dana Anggaran Pembelanjaan Belanja Daerah (APBD) di daerahnya.
Anak Oto, Gema pun menceritakan bahwa karena susahnya akses ke pembiayaan BPJS itu membuat ia dan ayahnya memilih pelayanan klinik untuk mendapatkan obat oral pengendali gula darah.
“Obatnya buat nurunin gula darah biar ke level normal. Normal untuk orang dibetes lah ya, bukan level normal kita (dengan gula darah normal tanpa diabetes),” kata Gema kepada Kompas.com, Rabu (15/12/2021).
Sebenarnya, lanjut Gema, ia dan ayahnya ingin bisa mendapatkan insulin. Tetapi, di sisi lain pemakaian insulin injeksi yang tidak mudah membuatnya berpikir ulang.
“Bokap gue (ayah saya) gak suntik, cuman minum obat minum (oral). Gak ada yang nyuntikin juga (kalau pakai insulin injeksi),” jelasnya.
